Summary

  • liaise with other significant people or organisations in the client’s life to facilitate the client’s goals and Action Plans
  • assess the level of risk to the client and others directly involved
  • follow organisational procedures, legal requirements and duty of care obligation in responding to indicators of risk of abuse, neglect or harm, including abuse or harm to children and young people
  • determine options for referrals to other service providers against the range of client needs
  • refer the client appropriately, following organisational protocols, policies and procedures
  • apply accepted procedures to evaluate the benefit to the client of referral to another service
  • evaluate the benefits of providing a brief intervention in helping the client access other services
  • apply accepted procedures to evaluate the option of bringing in specialist support and continuing to work with the client
  • provide the client with resources and information, appropriate to their situation and actions to be taken.
